Scaling TB elimination: High-yield community screening and detection strategies from South Africa's ACCELERATE program

Differentiated community-based TB screening and detection contributed to diagnosis and TPT initiation, highlighting the value of outreach beyond facilities. dCXR showed the highest test positivity (15.8%) and efficiency (NNT=6%). Contact tracing strengthened TPT uptake, while outreach expanded access. Findings demonstrate the value of integrated models. Scale-up should prioritize dCXR where feasible, alongside outreach and contact tracing. However, limited TPT initiation after dCXR and sputum collection challenges highlight the need for stronger systems and enhanced sputum collection strategies.
