Peak tuberculosis transmission at 24 weeks post-exposure with BCG-driven risk reduction in household contacts

TB transmission predominantly occurs within 24 weeks of exposure, suggesting this as the critical window for detection. Our findings support: (1) targeted IGRA retesting at 24 weeks for exposed contacts, particularly those with identified risk factors; and (2) continued BCG vaccination in TB control programs. This approach offers an efficient strategy for early detection and prevention in high-burden settings.
